Service Protocol.

Step One
Golden Care Dental Services’ Introductory Letter will be provided to the facility in sufficient quantity for mailing to all family / decision makers. This letter outlines the details of our program and the initial free screening date
for your facility.

Step Two
Our dental team will arrive at your facility and conduct a visual evaluation of all residents at no cost or obligation. This is entered in the appropriate “Progress Notes” and forms a baseline from which all residents are to receive appropriate dental care. Your facility will receive a copy of our findings.

Step Three
Golden Care Dental Services will then send an Estimate to the resident’s family / decision maker outlining our recommended treatment plan and its proposed costs. The family / decision maker will be required to forward a signed Consent Form to our head office before treatment can begin. No resident will be treated without signed consent.

Step Four
Once we receive the completed Consent Form our team will schedule the resident’s care. The facility’s staff will be advised of all scheduled appointments by fax prior to the date of treatment. Our staff then reviews medical history and treatment suitability and the attending medical doctor is consulted as necessary.

Step Five
Golden Care Dental Services visits the facility and treats the resident on the scheduled date. All necessary equipment is brought to the facility by our dental team. A resident can be seen in their room and, if necessary, while in bed. If your policy is to use a specific location, our equipment will be arranged accordingly.

Step Six
Golden Care Dental Services routinely visits your facility on an as needed basis, to treat consenting residents, to screen new admissions and to guarantee the success of our program. Golden Care Dental Services will continue to schedule appointments until all recommended treatment is complete.

New Admissions
At the time of a new admission the facility should assist the family in completing a Request for Screening form which is then faxed to the Golden Care Dental Services’ head office to initiate this process.

 

Working hand-in-hand with the facility we aim to create a goal oriented and non-invasive dental treatment plan for each resident.

Emergency Response
It is our commitment to provide efficient and timely dental care with ultimate regard for the safety and well-being of the resident.

1) Golden Care Dental Services is notified about the emergency.

This is usually in the form of a fax or telephone call and is initiated by the facility staff. It is essential that the facility have the family / decision maker contact our office directly. This ensures that the responsible party is aware of the emergency situation and is involved in the decision process to best suit the needs of the resident. This will include: possible treatment options, possible costs, and the possible avenues to provide care to the resident.

2) Determine the nature of the reported emergency.

This will include answering questions such as:
• Does the resident have natural teeth or dentures?
• What are the symptoms and how long have they been present?
• Is the medical history supportive of immediate dental care such as extractions or are there issues regarding current medication regimens (such as Coumadin or Actonel) that greatly compromise care?
• What is the co-operation level of the resident and is an oral sedative necessary?

3) Once a clear picture of the resident’s needs is clarified the necessary dental care can be arranged.

The following options will be presented to the family / decision maker in providing the best possible emergency care for the resident:

Option One - On-Site Care

An emergency appointment with a dentist from Golden Care Dental Services. This will be determined based upon the residents needs, suitability for treatment and the dentist's availability.

Option Two - Conventional Dental Office
Treat the resident within our conventional dental office (located in Scarborough). These appointments are typically arranged on a same day basis. Transportation to and from the office will need to be arranged by the family / decision maker.

Option Three - Hospital Referral

If there are over-riding medical considerations it may be necessary for the resident to be seen at a hospital with a dental team on-site.
